Important change to student course survey process

The student course surveys will now be processed by The Institute for Learning and Teaching. The new contact for the surveys is Amanda Purnell, (970) 491-2276.

Forms available at Morgan Library Help Desk

To get blank forms please contact the Morgan Library Help Desk, (970) 491-7276, 1st floor of the Morgan Library, west of the Loan Reserve Desk. Please request only one coversheet per course, per section. Also, please only request one survey form per student, per course, per section. Please do not stock pile.

The survey forms are orange. They are shipped to us by the manufacturer in reams of 500. We will distribute those forms in reams. You can request smaller amounts and we will break reams for you if you need less or do not wish to, or do not have the space to, store them.

The cover sheets are purple. Please request only what you will need for the semester. When counting, remember that you need one cover sheet per course, per section.

After the surveys are administered to the students

You can have them delivered directly to:

Please remove any department created additional papers before submitting the surveys for processing. This will speed up the preparations at this office.

Processing

Processing will begin immediately upon receipt of the surveys. Please feel free to begin sending them in right after they are administered. You do not need to hold them for any length of time. However, all surveys that are processed during the last two weeks of class and the week of finals will be held until the grades are posted. The forms will begin being mailed back the day after grades are due and continue mailing daily until all of the forms are processed.

Handbook procedures

Per the Academic Faculty and Administrative Professionals Handbook (see below), surveys will only be mailed back to the instructor being surveyed. If an instructor includes a home address on the cover sheet, they will be mailed to them at that address. Please do not ask to have them mailed to a department secretary as that is not allowed.

Hold any individual surveys that were not submitted with the rest of the course section. Please do not send them to have them inserted with the remainder of the course. There are too many course sections submitted to be able to match the individual to its respective course. Hold onto the form until the rest of the course is received back. Then either hand tabulate the survey into the results or send the entire course section back with a new cover sheet and the original results. The original results will be deleted and the entire course will be rerun to include the previously separate form.

After all forms have been processed, a report including all surveys administered during the last two weeks of class or the week of finals, and that do not have the "First Year at CSU" and/or the "Graduate Teaching Assistant" bubbles marked, will be posted to the ASCSU website.

SECTION I.8 Academic Faculty and Administrative Professionals Handbook

The Student Course Survey is designed to provide feedback to course instructors and is to be used for course improvement. It is one of the appropriate methods for including student input in evaluating teaching effectiveness as described in Section E.12.1. In addition, it is designed to provide information for students to make informed choices about courses. Each term, course instructors shall conduct a student survey of all the courses they teach through a system administered by the University utilizing the standardized University wide instrument. A quantitative summary of each course surveyed shall be forwarded directly to the course instructor, and the original forms shall be forwarded only to the course instructor. A quantitative summary of each course surveyed shall be released to the Associated Students of Colorado State University ("ASCSU"), provided that ASCSU contributes a fair share, not to exceed half, of the required financial resources to operate this program. Graduate Teaching Assistants and first year instructors shall administer the survey, but results will not be released to ASCSU. The Committee on Teaching and Learning and the Committee on Responsibilities and Standing of Academic Faculty are responsible for recommendations on the survey instrument, on reporting of quantitative results to ASCSU, and on other matters related to this policy.
