Classified staff may receive free flu shots Oct. 22

The CSU Human Resources office in conjunction with the State of Colorado Department of Personnel and Administration are delighted to announce a free flu vaccination clinic on campus for state classified employees and eligible dependents.

The clinic will be held from 3-5 p.m. Oct. 22 in Lory Student Center Rm. 228.

The event is sponsored by United HealthCare (UHC) and Kaiser Permanente, the state’s medical plan providers. A synopsis of eligibility criteria, event admission requirements and information about the health advantages of obtaining a flu vaccination are available in the October HealthLine publication and vendor links on the State of Colorado website.

Eligibility criteria

Employee, spouse, same gender domestic partner and children enrolled in a State of Colorado UHC (HDHP or Co-Pay Choice Plus) or Kaiser Permanente (HMO HDHP or HMO CoPay) plan may receive a free flu vaccination while supplies last.

  • UHC requires a minimum age of 4 for children
  • Kaiser Permanente requires children to be age 19 or older

Requirements

Insurance identification cards are required to attend the free flu vaccination clinic to ensure State of Colorado medical plan enrollment. If you forget you medical insurance indentification card, you will be required to pay $25 per flu vaccination.

A consent form must be completed to obtain services. Classified staff not enrolled in a UHC or Kaiser medical plan may participate in the flu clinic for a fee of $25 per flu vaccination
