Annual health screening for employees Sept. 28 & 29

The Colorado State University Benefits Office is sponsoring the annual health screening (blood draw) on Tuesday, Sept. 28 and Wednesday, Sept. 29 from 7-10 a.m.

Reduced employee cost

Free-Well, Inc. will conduct the event again this year. The health screening provides an opportunity to receive information about your health status through interpretation of your blood results at a reduced employee cost.

Free-Well, Inc. asks that advanced registration be completed to better accommodate your busy schedules and reduce the amount of time you may wait to obtain services. The instructions for this process are located in the brochure at

You will be asked to share the following information:

1. Your name (including the spelling)
2. Mailing address (to receive paperwork in advance)
3. Preferred appointment time
4. Telephone number

Please understand that the large number of employees, spouses, and domestic partners who attend this event will create lengthy wait times for those that choose to not register in advance. As a courtesy, advance registration is requested to ensure enough phlebotomists are on hand for this important event. Payments for screenings is due at the time of service (cash or check).

Results available Nov. 9

Your results will be confidentially distributed on Nov. 9, 2010, by a health educator who can help you interpret your test results. Please bring proper identification to ensure protection of your health information when you arrive to pick up your results in the Lory Student Center, Room 227 between 9 a.m. and 2 p.m.
